Whether it’s therapy, psychiatry, or other care options, Quartet can match you to the type of mental health care that fits your needs, preferences, and is covered by BCBSAZ.

  • If you are a Medicare Advantage member, please click Medicare Individual
  • If you bought your insurance through the HealthCare.gov website, please click Commercial Individual
  • If you receive your insurance through your employer, please click Portal Groups
  • or call Quartet directly at 877-258-4010 and press option 1
  • Fill out a short online form to be matched to the care you need
  • Quartet will match you with a provider in your area
  • If the provider isn’t the right fit, don’t worry — a Quartet Care Navigator will work with you to find your right match

Location Restrictions
If a member wants face to face interaction with a provider, they will need to live in or be willing to travel to Maricopa County or Pima County at this time. If a member lives outside either county, but willing to consider virtual appointments, they can still utilize the Quartet service.
