<strong>What is the Health Management Program?</strong></h5>
<p>
<span style="color: #000000;">It&rsquo;s a no-cost AZ Blue program where you can get personalized, guided support for up to 90 days via phone call, text, or video chat. The Health Management team is made up of nurses, respiratory therapists, registered dieticians, and certified diabetes care and education specialists who help eligible AZ Blue members live a healthier lifestyle if they are recently diagnosed with:</span></p>
<ul>

    Asthma</li>
    <li>
    Diabetes</li>
    <li>
    Chronic obstructive pulmonary disease (COPD)</li>
    <li>
    Coronary Artery Disease (CAD)</li>
    <li>
    Congestive Heart Failure (CHF)</li>
    <li>
    Or Chronic Kidney Disease (CKD)</li>
</ul>

<strong>What kind of support does the program offer?</strong></h5>
<p>
One-on-one health coaching, education on things like nutrition and medications, guidance on how to make the most of your health insurance benefits, tips that can help you feel better and save money, and <a href="https://www.azblue.com/inspire-health/blog/have-a-chronic-condition-take-advantage-of-az-blues-health-management-program" target="_self">more</a>!</p>

<p>One AZ Blue member living with <a href="https://www.azblue.com/diabetes" target="_self">diabetes </a>said the program was life-changing.</p>

<em>&ldquo;There is a daunting amount of information on being diabetic but having someone help me navigate the changes I needed to make was beyond helpful. [My Health Manager] was an incredible help and I felt she was genuinely invested in providing support and encouragement. I attribute much of my ability to get my A1C levels to a more manageable level to her help and guidance. Health Managers have engaged with nearly 1,500 members, and about 600 have graduated from the program so far.</p>

Health Managers reach out to AZ Blue members who are eligible for the program to see if they would like the extra support &ndash; for free!</p>
