AZ Blue PCMH Programs (Total Care)
Our AZ Blue patient-centered medical home (PCMH) programs support providers so they can enhance the overall health of their patients. It’s all about whole-person “total” care.
PCMH providers spend more time with patients during office visits. They help patients stay on track with preventive services and take advantage of wellness coaching. They work closely with patients who have chronic conditions to better meet their care needs.
We sponsor two PCMH programs:
- The Primary Care PCMH Program
- The OB/GYN PCMH Program
Both programs align with nationally recognized, evidence-based guidelines for patient care. We use these standards to set quality metrics that are tied to provider incentives.
PCMH providers can earn the Blue Cross Blue Shield Total Care quality designation. This national quality program recognizes providers and facilities across the country that are committed to coordinated, patient-focused, and in many cases more affordable, healthcare. To qualify for our PCMH programs, you must see patients in an office setting and agree to all requirements of the program. If you’d like to join us, you can start the process by completing an interest form.
