- Check the member ID card for the network name, then access the provider directory and find the provider’s listing. Click the “NETWORKS ACCEPTED” link to see the networks included in the provider’s AZ Blue contract.
- Visit the prior authorization and medical policy page to access our current code lists and the prior authorization lookup tool.
