  • For individuals and families, employer sponsored coverage, and Medicare members, log in to the AZ Blue member portal.

    For Medicaid members, you can log in or register through our Blue Cross Blue Shield of Arizona Health Choice member portal. 

  • An Explanation of Benefits (EOB) summarizes the care you received, the discount AZ Blue negotiated for you, how much your plan paid, and how much you may owe the provider. It is not a bill. We recommend checking this document with the provider bill to be sure you are billed accurately.
  • If you're an AZ Blue Medicare Advantage member, your plan includes prescription drug coverage also known as Part D. If you're a Medicare Supplement member, you will need to enroll in a standalone prescription drug plan. Click here to find a plan in your area.
  • If you have a balance, you will receive a payment slip with a self-addressed envelope in the mail to send in your payment.

    Only put one payment in the envelope. If your spouse also needs to make a payment, please pay each premium separately with one slip and one check per envelope.

    Payments cannot be made via the website or portal at this time nor be emailed.

  • Log in to the AZ Blue portal for claim status. A list of your claims is near the top of the page, then click on the claim for further details, including a process date and claim number.

  • SilverSneakers® gives members on select AZ Blue Medicare plans the opportunity to conquer their health goals. Follow these steps to get started with SilverSneakers®.

    1. Get ID number. Find your Member ID number at SilverSneakers.com/StartHere, through the SilverSneakers® GO mobile app, or by calling 1-888-423-4632.
    2. Find a location. Visit SilverSneakers.com for an interactive map showing participating locations.
    3. Get active from home. Members can log in with their ID number at SilverSneakers.com/StartHere. Here, you can access SilverSneakers® LIVE virtual classes, 200+ On-Demand videos and other exclusive member resources.
    4. Set a date. Take your ID number to the location of your choice or choose a day to start your virtual workouts.

  • Use the AZ Blue portal: Members can sign in to the AZ Blue portal. Once logged in, select the "Find Care" tool to locate in-network care options, including hospitals, urgent care facilities, and specialists.

    Provider Directory: You can also use the provider directory on AZBlue.com/medicare without logging in to search for in-network providers. You will need to select your network and plan information. For Medicare Advantage, a provider directory PDF is also available here.

    Check with the Provider: Members can contact their provider directly to verify coverage by calling the office or visiting the provider’s website.

  • You can find a copy of your ID card in the AZ Blue member portal. Log in here and locate the My ID Card link on the top right of the page. From there you can view or download your digital ID, order a new card or email it to a provider.
