- Live in the plan’s service area (Apache, Coconino, Gila, Maricopa, Mohave, Navajo, Pinal, and Yavapai counties)
- Are enrolled in Medicare Part A (Hospital Insurance) and Part B (Medical Insurance)
- Are enrolled in AHCCCS (Medicaid)
