Have a Chronic Condition? Take Advantage of AZ Blue’s Health Management Program

More than 4 million Arizonans have at least one chronic condition, which is an ongoing health issue that limits activities of daily living. Approximately 9% have three or more chronic conditions. Some of these Arizonans are struggling to manage their condition(s) and unique needs – and don’t know where to turn for help.

Whether you were just diagnosed or have an ongoing chronic condition – you can lean on us for guidance! Blue Cross Blue Shield of Arizona (AZ Blue) can help you navigate your health journey at no cost if you are living with any of the below chronic conditions:

• Chronic Obstructive Pulmonary Disease (COPD)
• Congestive Heart Failure (CHF)
• Coronary Artery Disease (CAD)
• Chronic Kidney Disease (CKD)
• Asthma
• Diabetes

AZ Blue’s new and expanded Health Management Program gives certain members who meet eligibility extra care, support, and encouragement to live a healthier life. What could that extra support look like? We have a team of licensed clinicians, like nurses and dieticians, who are ready to provide:

• One-on-one health coaching
• Education on nutrition, medications, medical equipment, and more
• Lifestyle change tips that can help you feel better, save money, and prevent future health problems
• An individualized plan of care
• Connections to community resources to support any non-medical, social factors influencing your health, like lack of transportation or lack of access to nutritious foods
• Coordination of care between members and provider(s)
• Referrals and transitions to other programs to prevent gaps in ongoing support

For example, if you were just diagnosed with diabetes and can’t figure out how to use your new medical equipment, like a blood sugar meter (glucometer), the Health Management team can give you some step-by-step instructions!

Your health is personal, and the AZ Blue Health Management team works to meet those personalized needs. You will work with your team of health professionals conveniently over the phone to address health questions or concerns. The program is intended to be between 30-90 days, giving you tools and skills so you feel empowered to take control of your health and confidently manage the condition on your own.

How do you know if you are eligible for the program?

Right now, this program is provided to federal employees and some AZ Blue members with employee-sponsored health insurance who have the above-listed chronic conditions. We are working to expand this program to help even more members with an even wider range of conditions.
