AZ Blue Health Choice Expands Access to Care with In-Home, Whole-Family Care Model Through Nest Health Partnership

For many Arizona families, getting to a doctor is not just inconvenient – it can feel impossible. Work schedules, transportation gaps, childcare needs, managing everyone’s health, and finding the right time for a doctor visit can be a lot to ask. AZ Blue Health Choice is working to change that – and we are bringing the care right to our families' front door.

Through AZ Blue Health Choice’s Medicaid team and its partnership with Nest Health, members can receive visits that address the entire family’s health needs in one appointment, right in their home.

In a single in-home visit, members can complete well-child exams, postpartum care, chronic disease check-ins, behavioral health screenings, and vaccine catch-ups for everyone in the household.

And when a family needs care at 2 a.m., AZ Blue Health Choice members can reach a clinical care line that is staffed around the clock. 85% of calls that would have ended up in an ER visit are resolved at home instead.

Arizona has a well-documented gap between health outcomes of Medicaid-enrolled families and the broader population. Access is a major driver – not just cost, but the practical barriers that make it hard for working families to maintain consistent care.

Families who previously had no regular care are now getting same-day appointments from a team that knows them. And with members reporting high satisfaction rates, it is clear that, when care comes to families on their terms, they show up for it.

A new mom keeping four kids healthy while navigating food insecurity.

Three months postpartum with three children and no primary care provider, this AZ Blue Health Choice member connected with the program while still waiting for SNAP approval. In one in-home visit, her family’s vaccines, screenings, and well-child visits were completed, postpartum depression screenings came back negative, and the care team helped her navigate SNAP enrollment. 10 quality care gaps closed in a single appointment.

A family that had been using the ER as their primary care.

Before connecting with AZ Blue Health Choice and the Nest Health partnership, this family had made seven emergency room visits as their only access point to care. Through the program, ER visits dropped to two. Mom is now managing her heart condition with medication and connected with a licensed social worker. Both daughters completed overdue well visits and vaccines.
