As shared in our previous newsletter, AZ Blue’s focus on value based care (VBC) supports better coordination, predictable access to care, and improved member experiences. One of our partner organizations working within this model is Arizona Priority Care (AZPC).

AZPC Overview

Arizona Priority Care is a local organization that supports a primary care centered approach to healthcare and partners with multiple Medicare Advantage plans. Their organization is designed to help align members, primary care providers (PCPs), and care teams around ongoing health needs.

Programs Available to Members

AZPC offers different programs for care coordination and support, including:

• Telephonic Care Management – Nurse outreach to answer health questions, coordinate care, and connect members to resources. •
• Hospital Discharge Support – Post discharge follow up to review medications, reinforce instructions, and support recovery after a hospital or ER visit.
• Palliative Care – Additional support for members managing serious or long term conditions, focused on comfort and quality of life.
• Medication Optimization – Pharmacist led reviews to support safe, effective, and affordable medication use.

Home Wellness Program

AZPC also offers a Home Wellness Program, which includes an in home assessment at no cost to the member. Visit results are shared with the member’s PCP to help with care planning. Members who complete a visit may receive a gift card to Walmart or Target as an incentive for participation.

Accessing Care

When members receive care within the AZPC network, no referral is required, helping members access specialty care with ease.
