AZ Blue members can earn up to $155 in rewards by completing annual wellness visits and other health screenings. Getting regular health exams and screenings can help detect health issues early. By completing an online form, members can earn rewards which are added to their AZ Blue Over-the-Counter (OTC)/Rewards Benefits Card Mastercard® and can be used at any location that accepts Mastercard. AZ Blue Medicare Health Rewards is available to members enrolled in one of AZ Blue’s Medicare Advantage health plans. Reward dollars cannot be used to purchase alcohol, tobacco, firearms, gift cards, or prescription drugs.
