If you’ve been recently diagnosed with a long-term (chronic) health condition, you can work directly with our care teams. They will help you set realistic health goals and stay on track to meet those goals.
Chronic Condition Management
Manage and improve your health.
- Work directly with a nurse, dietitian, respiratory therapist, diabetes educator, or behavioral health provider in the program.

Health Management
Get up to 90 days of condition-specific guidance to help you manage one of the following six chronic conditions:
- Asthma
- Diabetes
- Chronic obstructive pulmonary disease (COPD)
- Coronary Artery Disease (CAD)
- Congestive Heart Failure (CHF)
- Chronic Kidney Disease (CKD)
Clinical Rapid Response Team
If you’re facing serious or complicated health conditions and feeling unsure about what to do next, AZ Blue’s Clinical Rapid Response Team is here to support you at no extra cost.
Care Management
Some diagnoses require more compassionate support. Care Management integrates with Health Management to provide expanded access to resources. Care managers help you:
Understand what your health plan includes
Make the best choices for your care
Find special equipment like wheelchairs or walkers
Get referrals to in-network doctors you need
Get prescription costs in advance
