- Qualifying AZ Blue members can get clinical support and care management at no cost. Medicaid members will be connected to our care management team for support. If you have a health concern, you can contact us or fill out the form, and we will guide you to the right person who can help—no matter what plan you have.
- Health experts can assist you with conditions like diabetes, high-risk pregnancies, mental and behavioral health, chronic conditions, and major diagnoses like cancer.
- A member of our team will reach out to you and ask about your goals and the condition you need help with. We’ll discuss physical and mental health, support needs, and medications. Your health expert will work with you to create a plan that helps you meet your goals.
- Your time working with a health expert will depend on your goals and condition. You may be able to get help in one call or work together for several months. If you need support, you can always reach out for assistance. Additional help for new or ongoing conditions are available even if you’ve worked with a health expert before.
