Our ACA StandardHealth with Health Choice plan requires an assigned Primary Care Physician (PCP). Once the form is submitted, your PCP will be updated in 3 business days. You can confirm the update has been made by logging into your member portal.

For all other plan types, you can change your PCP by calling the Member Services number located on your ID card.
