Change Your Address
Once we receive your current address, you will receive your rewards card within 7-10 business days.
Be sure to also update your address on healthcare.gov. If you do not update your address with healthcare.gov, you may miss out on important health plan information.
For questions about rewards, call 1-800-960-5710. For questions about your health plan or your address, call the number on the back of your member ID card.
