Provider Overview
Our Philosophy
We are committed to a collaborative approach with physicians, hospitals, and all other providers in the medical communities we serve.
Our team brings an open vision to Arizona. We believe that those who provide care should be the leaders in creating and developing new, improved, and less invasive mechanisms for delivering the care they provide. We are provider-owned, and we understand both the rewards and difficulties of managed care and health plan/provider relationships.
We believe our members deserve the highest quality medical care while being treated with both compassion and respect. Assisting you so that you can devote your time to providing quality patient care is one of our highest priorities. Our commitment to you is to support the doctor-patient relationship by streamlining the delivery of care.
What We Offer Our Physician Network
We bring the expertise and road maps necessary to understand, participate in, and maximize the value of the sweeping changes affecting the delivery of health care.
We offer real-time tools, technology and up-to-date information to our physicians and providers. We will assist and offer guidance to physicians and hospitals to build partnerships, establish patient-centered medical homes, and other entities that maximize quality and reward performance.
Provider Escalation
We are committed to ensuring that you have an open line of communication with us at all times. Should you ever feel the need to escalate an issue, please send an email to Network Leadership at: hchaznetworkleadership@azblue.com

Once the Network Services Department receives the Request for Participation/Update Provider Information Form, we will evaluate and compare our membership needs with current network availability, along with several other criteria, including but not limited to: board certification, languages spoken, and office locations.
Once the previous items have been reviewed, the request moves to the Contracting Committee for review. If approved, the Network Services Department will contact you to initiate the Contracting and Credentialing process. We credential providers according to nationally recognized standards that serve as the model for minimum credentialing requirements. We do not discriminate against age, color, creed, disability, national origin, race, religion, sex, or sexual orientation.
Credentialing typically takes approximately 60 days from the date we receive a complete credentialing application and up-to-date attachments.
For additional information, contact the Network Services Department at 1-800-656-8991.
