Children should have a dental check-up every six months. You do not need a referral from your child’s doctor to see a general dentist or pediatric dentists. Children as young as one year old should have a dental check-up.

To find a dentist, please see the BCBSAZ Health Choice Provider Directory. You can also call Member Services, Monday – Friday, 8 a.m. – 5 p.m. at 480-968-6866 (Maricopa County) or 1-800-322-8670. We will help you find a dentist.

Dental services for members ages 21 and older are limited to emergency dental services to relieve severe pain and/or infection. Members 21 years of age and older can receive emergency dental services of up to $1,000 for each 12-month period beginning October 1st through September 30th.

BCBSAZ Health Choice also covers limited dental services as needed for members who are about to receive an AHCCCS-covered organ transplant and for members getting ready for radiation treatment for certain cancers (cancer of the jaw, neck, or head).

Dental Home Program

BCBSAZ Health Choice is pleased to announce our new Dental Home program. The Dental Home program helps members who are 0-20 years of age access dental care.

Dental Home program details include:

  • Members 0-20 years of age will be assigned to a BCBSAZ Health Choice contracted dentist.
  • Members will receive a letter telling them who their assigned Dental Home provider will be.
  • Members may see any dentist in the BCBSAZ Health Choice network.
  • Covered dental services will not be changed by this program. Members will still receive the same covered dental services.

BCBSAZ Health Choice thinks of you as a valued member. We want to make sure you get the services you need. Please feel free to call Member Services at 1-800-322-8670 if you have any questions or need more information.
