Know How Your Plan Works




Coordinated care is as easy as 1, 2, 3



1      Always go to your designated primary care provider (PCP). Your PCP is your main doctor and point of care. If you don’t go to your designated PCP, or a doctor who is covering for your PCP, you’ll be responsible for the full amount of your bill.*
2      If you need to see a specialist, ask your PCP for a referral. Your plan won’t cover most specialist visits without a referral from your PCP.
3      Staying in network can save you money. Remember, out-of-network providers aren’t covered by your plan, except in emergencies. This means you will have to pay their bill.
