Below are the benefit books and riders for the 2022 Zero Cost Share Individual & Family Plans.

The plans listed on this page are available during open enrollment, with effective dates on and after January 1, 2022.

Each listed plan has a link to the base benefit book, plan attachment, and any riders for that plan. A "rider" is a plan document that modifies the terms of the benefit plan, such as adding or removing benefits, and changing the member cost share. BCBSAZ can change the terms of a benefit plan or a rider as required or allowed by applicable state and/or federal law.

We have a lot of different plans, so make sure you are matching up the right base benefit book, plan attachment, and rider with the plan of interest to you. If you have any questions about the information in a base benefit book, plan attachment, or rider or believe you have accessed the wrong documents, please call Blue Cross Blue Shield of Arizona at (877) 475-8440 for help.
