Prescripton Drug Tiers
Blue Cross Blue Shield of Arizona has an open formulary. This means that almost all drugs are covered. The drugs are categorized in "Tiers". There are four Tiers. In Tier One, drugs are typically generic and usually the cheapest. Typically, costs of drugs increase as the Tiers get higher.
Least Expensive (Tier1)
Tier 1 generally consists of drugs with the lowest cost share. This Tier typically includes most generics. Although some more expensive generics may be on other Tiers.
Level 1 Generic and Level 2 Medication List
Mid-Range Cost (Tier 2)
Tier 2 usually consists of brand-name drugs for which generic equivalents are usually not available. These are more expensive than Tier 1. Tier 2 may also include generic drugs that the plan has determined to be more costly than their brand name alternatives
Level 1 Generic and Level 2 Medication List
Higher Cost (Tier 3)
Tier 3 usually consists of brand-name drugs that don't fall into Tier 2. Drugs in this category have a higher cost share than Tier 2.

Highest Cost (Tier 4)
Tier 4 usually consists of specialty brand-name drugs that are in the highest cost share level.
